Job Description

The University of Findlay (www.findlay.edu) is seeking a full-time 12-month Pharmacy Fellowship in Population Health in the Medication Therapy Management Telehealth Center. The position is for one-year, with a second-year option available to qualified candidates upon mutual agreement of sponsoring entities.

The candidate must possess a Doctor of Pharmacy degree must be obtained prior to start date; must be a licensed, registered pharmacist in the State of Ohio with no restrictions on license; must carry personal professional liability insurance at all times; be eligible to serve as a preceptor for pharmacy students in the State of Ohio; ability to work in and understand the academic environment; skill in program development and execution; demonstrated commitment to the principles of diversity; excellence in communication and presentation skills; excellent interpersonal and communication skills, and ability to work well with other pharmacists, health care providers and students; possess a personal NPI number; sign and abide by Employer’s Non-Disclosure Agreement; and have no pharmacy license restrictions imposed by the Ohio State Board of Pharmacy. Preference given to candidates possessing APhA Medication Therapy Management Certificate training; and experience with OutcomesMTM/Mirixia MTM platform desired.

 

Travel is expected for this position. A valid US Drivers’ license with a safe driving record is required.
